Making sure it is the joint
Not all hip pain comes from the hip joint. Pain from the joint itself is usually felt in the groin, and worsens after walking or activity. Pain on the outer side of the hip, over the bony point, more often comes from the gluteal tendons or the bursa — bursitis and gluteal tendinopathy — which is usually managed without surgery.

Which operation
- Total hip replacement — the femoral head and socket are replaced with a prosthetic ball and cup. It is rated among the top five elective operations for improving quality of life, with more than 90% of patients achieving significant pain relief.
- Direct anterior approach — a minimally invasive route to the same operation, passing between muscle planes rather than detaching muscle from bone. Generally preferred where it suits the hip; the posterior approach is used where that is the better choice.
- Hip revision surgery — for a replacement that has loosened, worn, become infected, or been affected by bone loss or fracture. Dr Clark’s UK fellowship was in revision arthroplasty.
